You know who's really at fault? The dumb clerk at the terminal. He set up a trap without consulting anyone, which forced the police to act upon it without proper set up. Then he was all scared and kept looking over at Stabler, which spooked the perp, which caused the chase, which led to the boy's death. Good job, dude.

Eliot was still picking fights and getting angry. Last episode it was Finn, this episode it was Olivia. The job is getting to him too much, he's unstable. I wasn't surprised when now it looks like he's leaving the show, maybe? Damn, what a shame, he's been such a staple of the show, him and Olivia are the main staples of the show for 7 whole seasons. He's a great detective, it'll be sad to see him go, but he needs to go, and get better. Damn, that job must be hell, investigating these types of crimes every day.
